I had a similar issue with my 2019 Mitsubishi Outlander Diesel last year. The symptoms were exactly what you're describing, rough shifting and grinding noises. Turned out my transmission fluid was critically low, which was causing the pressure problems. Took it to my local Mitsubishi dealer (after a bad experience with an independent shop). They found a small leak in one of the transmission lines that had been slowly draining the fluid. Besides the leak, they also had to flush the transmission system because some metal particles had accumulated due to the low fluid level. It was definitely a serious issue that needed immediate attention, low transmission fluid can cause major damage if not fixed quickly. Got it sorted before any permanent damage occurred, but the mechanic said I was lucky to bring it in when I did. Your symptoms sound spot-on with what I experienced, especially at that mileage.
