Hey, so I had something similar with my 2008 Toyota Hilux (although not exactly the same symptoms). The mechanic found that my EGR valve was faulty and stuck. My mechanic replaced the EGR valve and the car is now running perfectly, no more complications. Your problem definitely sounds like a stuck EGR valve.
