After I first noticed these issues on my Discovery, I actually tried cleaning the throttle body twice before going for a replacement. The cleaning would help for maybe a week or two, but then all the symptoms would come creeping back, especially that annoying jerking during acceleration. The fact that your problems are worse when the engine's warm is exactly what I experienced too. In my case, it turned out the internal components of the throttle body were wearing out, which is why cleaning only helped temporarily. Trust me, I tried to avoid the full replacement cost, but in the end, it was just postponing the inevitable. Given that your symptoms came back so quickly after cleaning, I'd honestly recommend just going for the full replacement.
