I had the same problem with my 2013 diesel Toyota Hilux and the signs were pretty similar to yours: twitching when accelerating, engine light, the whole nine yards. It turned out that the control of the gearshift intake manifold was actually defective. In my case, the mechanics had to remove it and replace it with a new one. The whole thing was definitely not a walk in the park, it was a pretty serious problem. Without this part working properly, everything goes haywire. So your gut feeling about the swirl flap control could very well be correct. I can only advise you to have it checked as soon as possible. Waiting any longer could lead to further damage.
