Hey there! I faced the exact same issue with my 2012 BMW X3 35i last winter. The exhaust flap actuator went bad, causing that loud cold start and loss of exhaust control you're describing. Got it checked at a garage and found out the actuator motor had failed, quite common on these models. The vacuum lines were intact, but the actuator itself was toast. Had to replace the entire actuator assembly. It's not a critical safety issue, but leaving it unfixed will keep triggering that error code and you'll be stuck with that loud exhaust permanently. The repair itself wasn't too complex, took about 2 hours of labor. Definitely get it checked soon though, as moisture can get in through the faulty flap and cause more issues down the line.
