I had nearly identical issues with my BMW X3 2012 diesel last winter. The symptoms match exactly what I experienced, constant glow plug light, black smoke, and cold start problems. After the diagnostic, it turned out the glow plug control module was corroded and two glow plugs were completely dead. Got it fixed at my trusted garage three months ago. They had to replace the control module and all four glow plugs to be on the safe side, as the remaining ones were also showing signs of wear. The job took about 4 hours. This wasn't a major mechanical issue, but definitely something that needed immediate attention to prevent further engine damage and poor performance. These symptoms usually point to glow plug system failure rather than just a single faulty plug. Would strongly recommend getting it checked soon, especially with cold weather approaching. The black smoke and failed emissions test are clear indicators that your fuel isn't burning properly. Based on my experience, don't try to limp along with this issue, it'll only get worse and could potentially damage other components.
