The symptoms you're describing match my experience perfectly, the power loss, rough running, and jerking were really concerning. Turned out two fuel injectors were defective and needed replacement. The problem was causing incorrect fuel mixture, which explained the emissions failure and rough running. Since it's a hybrid system, the issue was actually affecting both the conventional engine performance and the hybrid power delivery. Had to have all injectors tested, they found uneven spray patterns and leakage. The repair was pretty serious since they had to partially disassemble the fuel system to access and replace the faulty injectors. The job took about two days to complete but solved all the issues: smooth running returned, power was restored, and emissions came back to normal. Given your mileage and symptoms, I'd strongly suspect injectors. Make sure to find a shop that specializes in BMW hybrids, it makes a big difference in diagnosis and repair quality.
